GOAL NorthPoint seeks to reduce health disparities, improve health outcomes, and enhance the overall quality of life for residents of North Minneapolis by providing high quality medical, dental and mental health care, including integrated social services, with respect, dignity and sensitivity to a culturally rich and ethnically diverse community.
BACKGROUND NorthPoint Health & Wellness Center, (NorthPoint) formerly known as “Pilot City Health Center” was established in 1967. For over 40 years, NorthPoint has provided health and human services to the residents of North Minneapolis. NorthPoint is comprised of two sibling organizations: a community services nonprofit with a broad array of social and human services (NorthPoint Inc.), and a full service health center that includes medical, dental and behavioral health clinics (NorthPoint Health & Wellness Center). The two entities share an integrated mission and report to a common CEO and community board of directors. As a safety net provider and a federally qualified community health center, NorthPoint has a mission to improve the health and well-being of its North Minneapolis community. NorthPoint’s target population are the many underserved and un-served residents of Near North - low-income residents, those who do not speak English, recent immigrant populations, residents who are uninsured and those who suffer from chronic disease and health disparities. Particular priority is given to households with dependent children who are at-risk of abuse or neglect as a result of their parent(s) inability to provide basic needs such as food, shelter and healthcare.
SERVICES AND PROGRAMS NorthPoint Health & Wellness Center clinics offer a comprehensive array of primary care services including family practice, obstetrics, internal medicine, pediatrics, and specialty care. Adolescent services are provided on campus, at the North High school clinic and the Plymouth Youth Center (PYC) clinic location. In addition, NorthPoint provides full service dentistry and behavioral health services. In 2010, NorthPoint patients made over 84,000 visits to NorthPoint’s medical, dental and behavioral health clinics. Ancillary services available at NorthPoint include optometry, nutrition services, laboratory, a full-service pharmacy and radiology. Because many North Minneapolis residents face barriers to healthcare, NorthPoint also provides facilitative services consisting of transportation, culturally competent and multilingual staff, translation and interpretation, assistance in applying for public programs, and a sliding fee scale. Community health services are provided through our innovative Community Health Worker program and multiple outreach efforts such as: Healthy Start for pregnant women; diabetes support groups; Project Support, a behavioral health program to support adolescents involved with the Juvenile Justice System; TOPS, a peer education program at North High to prevent teen pregnancy; smoking cessation programs; Breathe Free North—a program to discourage smoking in homes and cars; chemical dependency assessments; charter school screening; dental education programs and many more. NorthPoint also houses a Hennepin County WIC clinic.
NorthPoint Inc. (Human Services) provides a continuum of services designed to meet basic needs and promote health and self-reliance. It has an extensive community outreach program, is home to one of the largest food shelves in Minnesota and provides family strengthening services. In 2010, NorthPoint’s Human Services division provided food and advocacy to more than 30,000 individuals from over 10,000 households. Key programs and services include: Community Food Shelf, Housing Counseling and Emergency Housing Grants, Budgeting Assistance, Credit and Debt Classes, Advocacy, Community Outreach, Gateway Project (case management for families on public assistance), and The African American Men Project (AAMP). NorthPoint Inc., like the Helath Center, also provides supportive services consisting of transportation, culturally competent and multilingual staff, translation and interpretation, and on-site assistance in applying for public programs.
