Guiding Patients and Families

MNHPC provides materials, resources, educational forums, a helpline, and website for the public, raising awareness about hospice and palliative care, and advance care planning.

Serving Our Veterans

The Minnesota Hospice Veteran Partnership and our We Honor Veterans program allows MNHPC to provide training and resources to assist the VA and Minnesota’s hospice and palliative care providers to best serve the men and women who have served our country.

Increasing Access for Minnesota’s Multicultural Communities

MNHPC is currently developing culturally appropriate tools and resources for Community Health Workers and Allied Health Workers serving Minnesota’s multicultural communities. MNHPC also provides education and resources to hospice providers serving diverse cultural communities.

Educating Hospice and Palliative Care Professionals

MNHPC offers education for healthcare professionals, enhancing the care patients and families receive from the hospice and palliative care teams.

Advocating for Hospice and Palliative Care

Your support ensures hospice and palliative care is accessible for Minnesotans by enabling MNHPC to provide educational materials and advocate on the state and national level.
