We Help You Help. Whether you are taking care of an aging person or trying to manage your own health care, Eldercare Partners gives you the tools you need to organize and cope with your personal situation. We help you help your loved one - and yourself.
Eldercare Partners is a nonprofit organization uniting three of the most respected nonprofit organizations in the Minneapolis-St. Paul metropolitan area of Minnesota to provide award-winning Caregiver Services as well as Geriatric Care Management to seniors and their family caregivers.
Our Mission:
Eldercare Partners improves the lives of older adults and their family caregivers by providing high quality, easily accessible, community-based services through the collaborative efforts of our member organizations.
Our Vision:
Eldercare Partners is recognized as the premier nonprofit resource for community-based services for elders and their family caregivers living in the Twin Cities metropolitan area.
All donations are received and receipted by Razoo Foundation, a registered 501(c)(3) charity that operates a donor advised fund to fulfill donor advisements.
