A community collaborative working to enhance end of life and palliative care in Cook County.
An out-growth of years of work to create a hospice program in Cook County, Care Partners has developed a simpler approach and broader mission. North Shore Health Care Foundation, Cook County North Shore Hospital, Sawtooth Mountain Clinic and other community service organizations are collaborating to establish a palliative care program, supporting those with advanced illness, at the end of life or the frail elderly.
All donations are received and receipted by Razoo Foundation, a registered 501(c)(3) charity that operates a donor advised fund to fulfill donor advisements.
